Sage Wolf Named RCS Director Of Integrated Health
Redwood Community Services is happy to announce that Sage Wolf has been named the agency’s Director of Integrated Health.
Sage has been with RCS since 2007 and has held numerous roles within the agency, most recently as the Homelessness & Housing Support Manager at Building Bridges Homeless Resource Center.
In their new position, Sage will continue to work closely and help guide B2 while also pushing forward with enhanced care management through the CalAIM. More information about CalAIM can be found at DHCS.ca.gov/calaim.
“The part that is new (for me) is the enhanced care management through CalAIM. With CalAIM expanding out this whole person care model that’s been piloted in Mendocino County for the last six years or so, that is being expanded out to be more available and accessible to partnership beneficiaries,” Sage said.
Essentially, enhanced care management is utilizing the Whole Person Care model of care for people. This broadly includes those with high needs and vulnerable populations, such as those who have comorbidities such as substance use disorder, mental illness and physical health needs, Sage said.
Within this program, a care coordinator will be assigned to a client and will help guide them through the various service options available to them. One of the biggest benefits to this approach to treatment is its emphasis on integrating different service offerings together.
“That care coordination, to be able to first just get them to connected to the right resources and then make sure those resources are talking to each other,” Wolf said. “The more cohesive that we can get, the more integrated we can get with our care, the better we’re doing by the people we serve.”
Wolf will spearhead the effort to implement this program fully in Mendocino and Lake Counties, before expanding to Humboldt County.

Sage has worked tirelessly to improve lives in the community at since they first entered this organization and this change will help us as we constantly continue to improve our standards of care. Their energy, experience and enthusiasm makes them a natural fit for this position,” said RCS Chief Operating Officer Tawny Bailey.
